The thing that strikes me every single time I listen to First Spell is how the music can sound so beautiful and soothing while also having a very dark, hellish vibe with a strong aura of melancholy throughout. Keyboards in black metal have been used for decades, but often don't blend well or are just overbearing (if not downright annoying). Here Gehenna seem to have found the perfect balance, using the keyboards pretty much to perfection in their songs. The music doesn't sound all that extreme if you compare it to most of what was being done in black metal in Scandinavia back in 1994, with the raspy vocals arguably being the least accessible part. It's the atmosphere created by all these things put together that make this sound "black" and extreme, which is no mean feat.
